Dacryocystitis is the inflammation of the lacrimal sac that requires surgical treatment. The present paper is aimed at comparing the postoperative evolution in outpatients and inpatients, taking into consideration the complications, the satisfaction degree of the patient and the institutional costs in each group. Descriptive prospective study of 202 patients operated on of dacryocystorhinostomy divided into 2 ambulatory and hospitalized groups from January 2000 to April 2002. The surgical results and the institutional and patient's costs were evaluated and compared in both groups. 56 % of the inpatients and 48.03 % of the outpatients were over 61. Females predominated in 57 % of the inpatients and in 66.6 % of the outpatients. 97 % of the hospitalized patients and 97.5 % of the ambulatory patients had no complications. The mean of the institutional cost was 284 pesos per operated on patient. 93.7 % of the inpatients preferred ambulatory surgery and 100 % of the outpatients felt satisfied. Both groups of patients presented a similar distribution by sex and age. The absence of complications prevailed in both groups, as well as the surgical success. 32 400 pesos were saved by performing the ambulatory surgery
