The systemic lupus erythematosus is the autoimmune disease that complicates pregnancy more frequently. Some of the controversial topics that have been dealt with during decades are: the possible increase of the disease activity, whether gestation causes irreversible lesions in patients with nephritis, the influence of lupus on the poor obstetric and perinatal results, and whether a pregnancy is recommendable in a patient with lupus. A literature review was made that included the last series published about the effect of pregnancy on lupus, the impact of lupus on gestation, the fetal affection and the therapeutic options. The prognosis of a lupus pregnancy is good at present, provided that it is planned in a period of inactivity of the disease. The obstetric results are still reserved (increase of preeclampsia, preterm delivery, retarded intrauterine growth, fetal death), but they have improved compared with previous decades.
