Introduction: the current increase of pregnancy in middle aged women carries an higher risk for women during pregnancy, due to, among other factors, a higher incidence of preexisting conditions. Objectives: to identify key demographic and obstetric characteristics of pregnant women of advanced age, to determine the main causes of maternal morbidity in this risk group and to compare them with a control group. Methods: a retrospective and case-control analytical study was conducted on the risks of pregnancy in pregnant women older than 35 at Falcon Polyclinics from January 1st. 2005 to 31 December 2007. The control group consisted of a random sample of pregnant women from 20 to 34 years. Statistical tests and risk analysis were applied. Results: smoking was found in 71.43 % of the pregnant women of advanced age, who besides were overweight (42.86 %) and obese (38.09 %). In the study group, chronic hypertension and gestational hypertensive disorders were present in 57.14 % and 42.86 % respectively. Chronic hypertension (p = 0.000) and preterm labor (p = 0.021) were statistically relevant. Conclusions: smoking, excess body weight, chronic hypertension, and preterm labor were more frequent in the study group.
