The intestine has caught the attention of researchers since it acts as an "engine" for sepsis and multiple organ dysfunction syndrome in multiple trauma, burned, shocked patients, among others. Bacteria and toxin translocation is the responsible for the process that worsens the condition of these patients since bacteria and toxins encourage a generalized inflammatory response in the host. The main physiopathological events unleashed by the inefficient barrier function of the intestine were also discussed. Also the clinical tests used to measure barrier function and the state of oxygenation in the intestine were presented and the current microbiologic diagnostic means to determine translocation such as microbial DNA detection were described. Criteria on energetic surgical measures to prevent and counteract translocation and others such as nutritional support, better perfusion and other measures which are currently under controversial discussions regarding their effectiveness or are on experimental phase were also presented. The importance of all these methods for daily surgical practice was assessed.
