The stromal origin gastric tumors are infrequent. This is the case of a female patient presenting with a chronic pancreatitis history coming to our service due to epigastric pain verifying the presence of an abdominal tumorous mass. Imaging studies (abdominal ultrasound, esophagus contrasted study, stomach and duodenum and computerized axial tomography) allowed be interpreted as a stomach intramural tumor. Patient was operated on removing a large tumor mass adhered to epiploon, pancreas tail, spleen and stomach body. The pathology report was a large intramural "fusiform stromal tumor of gastrointestinal tract" with a metastasis high risk and mitotic activity in number of 10 x 50 high power fields with necrosis and hemorrhage and fistula in gastric mucosa. The diagnosis of the stomach stromal tumor is complex and the intramural lesion described by clinics and imaging studies was verified with this diagnosis by microscopic report from pathological anatomy.
