The direct intraosseous access for hematopoietic cell transplantation was first used by Josefson in 1934, as a method to treat pernicious anemia. Later, the use of tintravenous access became normal, based on knowledge of hematopoietic cells migration to the bone marrow once injected into the systemic circulation. The transplantation of blood cells from the umbilical cord has developed significantly in recent years. Among its disadvantages is that, generally, the number of cells obtained are insufficient, especially for using in adult patients. Therefore, it is important to increase the number of cells injected and to reach the core. One way to do this is by placing them directly into the marrow space. Preclinical studies have shown the effectiveness of this approach. Clinical studies are scarce and with conflicting results, so it is necessary to continue developing this method which appears to be useful in certain situations.
