The case of a 22-year-old patient addict to cocaine since adolescence is reported. Her first pregnancy and delivery took place 3 years ago. She had a child that  weighted under 2 350 g resulting from a cesarean section with acute fetal suffering. During gestation, she rejected the product of conception and after delivery she showed certain interest in her child during the first days of puerperium, but later she totally rejected the baby and began to leave him with neighbors and friends to consume cocaine.Several efforts have been made to detoxicate her, but she she has refused saying that if she does so, she no longer be herself. She  attempted suicide once. She began to work, but she had another relapse with deterioration of her person and quality of life, and she has not been able to recover. The child is most of the time with neighbors and friends and in children's institutions. He is very restless, presents stammering and day enuresis, he does not obey orders and he shows  retardation in some intellectual processes, he has little general information and he has no muscular control. In general, he needs affection and social approval.
