An observational prospective study planned for five years with the objective to determine the maternal and perinatal impact of diabetes in pregnancy since 1985 that continue to date was performed. The universe of study was constituted by all pregnant diabetic women at “Ana Betancourt de Mora” Teaching Provincial Gynecobstetric Hospital to ensure a correct diagnosis and follow-up according to their needs. In these 21 years, 1995 diabetic pregnancies were reported, 347 pregestational and 1510 gestational. Pregnant women with more than 30 years and those who had one or more deliveries predominated, a third part was classified as overweight and a fourth of them had excessive weight gain. Dystocic deliveries were about the 42.1% and the 19.9% primitive cesareans, low birth weight was present in 8.3% and macrosomic in 14.69%. Eighty-two newborns at birth presented depressions to the first minute of life and to the five only 22. It is necessary an opportune diagnosis of diabetes with a proper treatment and follow-up to obtain similar or better results than in normal pregnant women.
