Introduction: complicated acute appendicitis (gangrenous or perforated) is the most common cause of secondary peritonitis in the pediatric age. Recently, the sequential use (intravenous/oral) of antimicrobials in the post-surgical treatment of children with complicated acute appendicitis has been allocated a level B of evidences. The present work shows the present state of the knowledge regarding the sequential use (intravenous/oral) of antimicrobials in pediatric patients with the disease. Method: a review of the data bases of HINARI, Medline, PubMed, and Scielo was made. One hundred and fifty-six works and two books related to the subject were found. In the classification of the information, the practical guide known as Pyramid of the 6s was used. Development: in the review, 24 works and 2 books were selected because of their importance. The found results permit to asses some positive and negative aspects related to the sequential use of antimicrobials in pediatric patients with this disease. Among the positive aspects stands out the reduction of the hospital stay and as a side effect the reduction of the related costs. Conclusion: nowadays, although it is not possible to state unquestionably that the sequential use of the antimicrobial treatment (intravenous/oral) is the best option for children with complicated acute appendicitis, the described advantages for the use of antimicrobials taken orally and the existence of an adequate level of scientific evidence, permit to consider the possibility of its use.
