A study of an acute viral hepatitis A outbreak that occurred in a day care center in the City of Havana during 1996 is described. With the purpose of comparing their usefulness for diagnosis: clinical behavior, the level of the ALT enzyme, detection of viral antigen in feces and the presence of anti HAV IgM were considered. The relationship between these indicators was established. The doctor of the institution provided the information on the clinical behavior and the ALT values of the individuals. The detection of viral antigen in feces and the anti HAV IgM in serum were performed by ELISA. An 87.5% of the infected individuals remained asymptomatic and in more than half of the cases serum ALT was considered within the normal range. Only 3 of 84 fecal samples were positive to the viral antigen. The detection of anti HAV IgM showed the highest percentage (12,4%) of positive samples when compared to the other indicators. This correlates with the known fact that this is the choice method to establish the diagnosis of hepatitis A outbreaks.
