Introduction: Maternal deaths have gone decreasing throughout years, that is why the analysis of extremely severe maternal morbidity, with a new epidemiological view, can has a significant interpretation in its study for our context, as well as, contributes to the strengthening of medical care in order to reduce mortality. Objective: To characterize extremely severe maternal morbidity in Villa Clara, from January to June, 2012. Methods: A descriptive study was carried out; according to inclusion criteria, the universe was constituted by all expectant mothers diagnosed with extremely severe maternal morbidity in hospitals with obstetric and gynecologic services. To evaluate quality of hospital and prenatal care by means of model of request tactic evaluation. Information was obtained from medical charts in primary and secondary care and from the observation guides for each dimension. Results: A number of 20 patients were diagnosed with extremely severe maternal morbidity, average age 28 years, and there were identified maternal risks in 75 % of them; hypertensive disorders, vaginal infection, anemia and advanced maternal age were the most frequent risks. Morbidity was more related with the management of patients, and there were identified deficiencies in prenatal care, human resource training, application of color code model and some performance protocols, as well as, follow- up of patients in secondary care. Obstetric hemorrhage was the main cause of extremely severe maternal morbidity.
