Primitive tumours of the spleen are relatively uncommon, and among them, hemangiomas are the most common benign neoplasm in adults. We present a 22-year-old white-skinned patient with a history of high blood pressure. The patient reported discomfort in the left hypochondrium and abdominal ultrasound detected splenomegaly and renal lithiasis. Diagnostic and therapeutic splenectomy was indicated given the medical approach of hypersplenism. The main and most important complication that can originate this condition is the spontaneous rupture of the spleen in a quarter of the reported cases, usually in the large ones. The treatment of choice is splenectomy, mainly to avoid the risk of splenic rupture.
