An instrument for the study of the organizational environment in health institutions was proposed in this paper. Also reflections on different ways to determine the organizational environment and how to improve it were expressed. It is based on the importance of incorporating the Organizational Climate Inventory of PAHO / WHO, the communication dimension with its four categories: organizational development stimulus, contribution to the organizational culture, feedback process and styles of communication, all of which are measured through five subsections by category. This paper also presented the classification key for the instrument.  For the interpretation of the results, an ordinal scale for the correct answers that classify organizational environment: satisfactory, at risk and unsatisfactory was defined. The authors considered that the proposed instrument for the study of the organizational environment in health institutions provided managers with valuable information about the processes that determine the behavior organization for changing management in a planned way and in response to identified needs by the organization
