A cross-sectional study was conducted among 22 diabetics (18 type 2 and 4 type 1) over 15 that were affected by malignant otitis of the diabetic in order to know the clinical characteristics and evolution of these patients. A complete medical history (endocrinologist) and a detailed otological history (ear, nose and throat specialist) were made. Age, sex, type of treatment and time of evolution of diabetes mellitus, mean values of glycaemia, the result of the bacteriological study of otic secretion and of the imaging study, the symptoms and signs depending on that otitis, as well as the complications and the treatment outcome were determined. An intensive insulin treatment and a specific antibiotic therapy were used. The Pseudomona aeruginosa was the most common causal germ. Otalgia, the granuloma of the external auditory meatus and the compromise of the cranial pairs (VII, IX and X) were frequent. It was proved that more than half of the patients were better or cured at discharge. There were no deaths. Sequelae were observed in 4 patients. The malignant otitis of the diabetic was more frequent in patients with type 2 DM with an evolution of more than 10 years. The most common cause of surgery indication was the granuloma of the EAM. Sequelae were observed in 4 patients (18.2 %). The early diagnosis of malignant otitis of the diabetic, the intensive insulin treatment and an adequate antibiotic therapy may produce a significant reduction of hospital stay, sequelae and mortality in these patients.
