Introduction: children who present a complicated paraneumonic pleural effusion are seriously sick; the recovery from multiple complications that are usually present during the disease is  slow, this period can last a year and they may develop chronic diseases. Objective:  to assess the evolution of children with the diagnosis of complicated paraneumonic pleural effusion, a year after been discharged from hospital Method: a series of cases in 38 children that were discharged from hospital with complicated parapneumonic pleural effusion that underwent toracoscopia between January 2008 and January 2013 at Holguín Pediatric Hospital, Cuba. Results: at discharge, 17 patients (44.7%) presented malnutrition and a year before seven patients (18.5%) still presented malnutrition. The radiological discharge was possible after 39 days in 27 children and after 50 days in six children. Hemoglobin results were not adequate at the moment of discharge in 73.7% and it persisted during six months in 14 children (36.9%). Seven patients presented allergic respiratory disease; three respiratory intense infections had equal children's number over. Respiratory functional tests were normal. Conclusions: malnutrition and amounts of non-adequate hemoglobins are the complications of highest incidence in the later evolution it could be obtained to the certificate of discharge, the radiological certificate of discharge in most of the patients before the two months; the presence of allergic respiratory disease is an affection that is frequently present in  these patients.
