A retrospective study of 54 patients with multiple aneurysms in the circle of Willis, who had been operated on from January 1985 to December 1996 in the Neurosurgery Service of «Hermanos Ameijeiras» Hospital, was performed. Angiographic studies detected 121 sacs of which 115 were surgically treated in one attempt and 4 in two attempts. Two intracavenous asymptomatic sacs not approached. The approach of 23 bilaterally located aneurysms was unilateral for 19 cases (82.6 %) and bilateral in 4 (17.4 %). A total of 115 sacs were clamped and 4 reinforced. It was determined that 41-50 year age group (33.3 %) and females (75.9 %) were the most affected. The association of bilaterally located sacs in the anterior circulation was the most observed association (38.9 %). It could not be proved that the results dependen on the type of association but the highest mortality was recorded in anterior and posterior saccular combination and in patients with multiple unilateral sacs in the anterior circulation (40 % and 13.3 %). It can be inferred from this that the unilateral approach does not have a negative impact on the results achieved in patients with aneurysms at both sides of the midline. General mortality was 9.25 % and surgical mortality amounted to 5.5 %
