Transthoracic fine needle aspiration biopsy is an old method that sometimes is forgotten in the study of pleuropulmonary lesions. The objective of this paper is to evaluate the effectiveness of this diagnostic technique. 182 medical histories of patients with lung cancer were analyzed from 1987 to 1999. All the cases were diagnosed by puncture. 81.3 % of positivity and 18,7 % of diagnostic error by false negative were obtained. The most frequent histic type was the epidermoid carcinoma, 50,5 % and the adenocarcinoma, 31,3%. All the lesions were peripheral. Pneumothorax was the commonest complication in 65 % of the cases. There was no mortality attributable to the procedure. Thorax fluoroscopy was the most used complementary, 90 %. It was concluded that fine needle aspiration puncture is an ideal method in lung peripheral lesions and that it is the first step in the study of pulmonary opacities
