In the 1960s, the concept of perioperative prophylaxis was introduced and revolutionized the criteria of antimicrobial treatments existing up to that moment. This method allows to reduce the phenomena of perioperative sepsis and high hospital cost and to improve the quality of the attention given to the patient. The economic advantages of the cost-effectiveness relationship in the use of cefazolin for the perioperative prophylaxis of acute appendicitis and hysterectomies are shown. The cost-effectiveness analysis for colon surgery is in the same range of values of the indicators used. 104 patients treated with cefazolin and 107 treated with ceftriaxone underwent a prospective clinical evaluation at "Joaquin Albarrán" Clinical and Surgical Hospital, in Havana City, during a year.
