A bibliographic review of the economic evaluation studies in the antiretroviral treatments for HIV/AIDS conducted in the world was made in order to assess the economic and social benefits in terms of health for the patient and to compare them with the economic amount of the pharmaceutical resources used in the global treatment of this disease. Thus, the required levels of pharmacotherapeutical efficiency justifying the use of drugs in the health budgets of all the countries of the world that have to fight against this pandemia may be obtained. For this reason, pharmacoeconomics applied to the field of pharmacotherapy for HIV/AIDS is considered as another element contributing to improve the rational prescription of antiretrovirals, which leads to the efficient use of health resources. The cost of the treatments with antiretrovirals is considerable, but its assessment in terms of efficiency allows to know the more effective combinations and the best tolerated for the patient's health, defining the best treatment strategies and how they are influenced by the changes in the viral burden and by the appearance of resistance. That's why the rational and efficient use of antiretrovirals makes possible a drop in the consumption of hospital resources at the expense of a decrease of the length of stay, as well as an evident benefit of the reduction of morbidity and mortality of the patients treated with these drugs. As it is showed in the bibliographic review, the results obtained in the international pharmacoeconomic studies indicate that the antiretroviral treatments, mainly the extremely active therapy, have a good cost-effectiveness relation and that they should be offered to all the HIV/AIDS cases that could benefit from it, taking into account the obvious sign of the benefitial effects showed by this pharmacotherapy from the economic point of view and the clinical, immunological and virological results observed in the patients.
