The economic and social impact in terms of health due to the use of foreign antiretrovirals for a year was determined and a comparison with the absence of this treatment was made. Another aspect was the comparison as regards costs and efficiency of these treatment schemes, with the incorporation of antiretrovirals of national production and under the premise of presenting a similar effectiveness. A retrospective study was conducted in 59 medical histories for determining the effectiveness, measured by the survival of patients and expressed by the year of life gained, as well as by their clinical and immunological improvement. The expenses resulting from the use of the pharmacotherapy and hospitalisation were estimated and the economic evaluation was carried out by techniques of the cost-effectivity analysis and of cost reduction for different treatment alternatives. A survival of 96.6 % of the patients and favorable results of 81.4 % in the clinical and immunological improvement were attained for a high average cost of US $ 5 523.7/patient, for a cost-effectivity ratio of $ 5 717.6/year of life gained and of $ 6 789.6/case improved. The use of foreign antiretroviral agents brings about health benefits, but. increases the costs of pharmacotherapy, an aspect that is against the treatment efficiency. Therefore, the economic convenience of the national production of antiretrovirals is confirmed, since this health benefit may be obtained at a more reasonable cost for the country.
