The case of a 68-year-old patient with relapsing polychondritis, affectation of the VII cranial par during the evaluation and associated diabetes mellitus was presented. It was confirmed that the patient met all the diagnostic criteria of the disease, although it was necessary the biopsy of the cartilage of the ear for having unilateral affectation. He also had fever during 40 days and the respiratory sepsis got worse, but it was controlled with antibiotics of wide spectrum. He began to improve even without steroid treatment and he recovered completely on using it.
