A case with diagnosis of seropostive rheumatoid arthritis and a year of evolution was presented. The patients was treated with non-steroidal anti-inflammatories and methotrexate. In spite of this, the inflammatory articular activity was maintained at the level of the carpus and knees and there was a volume increase in the posterior region of both legs, with spontaneous pain and positive Homan´s sign. At first, it was evaluated the possibility of a thrombophlebitis of the popliteal region, but, finally, it was considered as a Baker´s cyst.
