The case of a 27-year-old female white patient with family history of diabetes mellitus and with irregularity of her menstrual cycle, frequent episodes of urinary sepsis and proteinuria in a non-nephrotic degree was presented. Diabetic retinopathy was not confirmed by the medical examination made on admission. Glycaemia and PTG were also normal. The renal function measured by glomerular filtrate was 60.6 mL/min. It was made a renal biopsy that showed diabetic glomerulosclerosis. The clinical evolution was characterized by isolated episodes of urinary sepsis and sporadic pictures of sweating and fatigue. Proteinuria was under lg/24h and, occasionally, she had elevated figures of glycaemia (9.3-16.1). She was put on a diet and ACE inhibitors were used. The renal function proved to have a slow but progressive declination. During her last visit to the physician´s office it was observed a clearance of endogenous creatinine of 40.2 mL/min. It was confirmed what is stressed in some international reports that diabetic glomerulosclerosis may be the initial clinical finding in a small group of diabetic patients who were not diagnosed until then
