The correlation between the arthroscopic image and the histology of the synovial membrane may be useful in the diagnosis of articular inflammatory pictures of nondefined cause. A prospective cohort study that included 23 patients with knee synovitis without defined diagnosis was conducted. Arthroscopy was performed to correlate the macroscopic and microscopic findings of the synovial membrane that allow to detect rheumatoid arthritis (RA) in case the criteria were not fulfilled. The characteristics of the synovial membrane were determined and biopsy was performed for histological study by hematoxilyn and eosin. The cases were observed during 6 years to identify those fulfilling the classification criteria for RA and the characteristics of the synovial membrane related to them. Chi square and Fishcer tests were applied for the statistical analysis. It was found out that of the 23 cases, during the follow-up, 20 met the criteria of the disease. The arthroscopic view of the synovial membrane showed that the presence of deposits of fibrin among the proliferated hairiness was the characteristic that in a statistically significant way was associated with the definition of RA diagnosis. From the histological point of view, the finding of lymphoid follicles approached to the statistical significance. This characteristic was absent in all the cases among whom the diagnosis could not be defined during observation. It was concluded that the correlation between the arthroscopic findings and the histology of the synovial membrane may help to suspect the RA diagnosis in patients that do not comply with the criteria yet.
