The case of a 63-year-old male patient that began to have fever of 38- 39°C every day without chills four months ago is reported. He was admitted in “Camilo Cienfuegos” Hospital, in Sancti Spiritus, where he underwent multiple investigations, but the diagnosis could not be defined. He was referred to “Hermanos Ameijeiras” Hospital with the diagnosis of fever of unknown origin (FUO). During the interwiew, it was found that he had lost 20 pounds of weight. The physical examination showed pale mucosas and hepatomegalia of 4 cm . After several investigations that included bone marrow biopsy on two occasions, it was suspected a chronic myeloproliferative syndrome without myelofibrosis, with a slight increase of the reticulin. It was decided to perform laparotomy with splenectomy and wedge liver biopsy that showed megakaryopoietic myeloid metaplasia in both organs. It was concluded the diagnosis of agnogenic myeloid metaplasia in a prefibrotic stage that presented itself by a fever of unknown origin.
