The case of a patient that was admitted in "Hermanos Ameijeiras" Hospital with primary antiphospholipid syndrome and history of deep venous thrombosis in the lower limbs during his youth, severe pulmonary hypertension, severe tricuspid insufficiency, hepatopathy and chronic colecystopathy was presented. In the high resolution tansthoracic echocadiogam, there were detected irregularities in the endocardial edge mimicking cilia that project towards the free ventricular cavity, located on the surface of the left ventricle,which is anatomically smoother at the level of the free wall and of the left ventricle outflow tract. The etiology may be related to the autoimmune activity of the antiphospholipid antibodies on this surface that cause these irregularities and that later could be the settlement sites of the multiple intracavitary thrombi reported in these patients.
