SUAREZ TRUEBA, Bettsy; HART CASARES, Marcia; ESPINOSA RIVERA, Fidel  e  SALAZAR RODRIGUEZ, Daniel. Identification of resistance mechanisms in multidrug-resistant clinical isolates of Klebsiella pneumoniae. Rev cubana med [online]. 2012, vol.51, n.3, pp.228-238. ISSN 0034-7523.

Introduction: the growing resistance of enterobacteria, particularly Klebsiella pneumoniae, is a current health problem, due to the high percentage of isolates in clinical samples. Methods: astudy was conducted of all the strains of Klebsiella pneumoniae isolated from critical patients hospitalized between 1 March and 31 December 2009. The strains were identified following the methodology in the automated system VITEK 2 compact (bioMériuex, France), as well as the manufacturer's recommendations, using ID GN cards for identification and AST N86 cards for sensitivity tests, both with automated reading. Results: multidrug-resistant Klebsiella pneumoniae was isolated in 29 patients, 51.7 % of them from blood samples and 89.6 % in intensive care units. High percentages were obtained of resistance to cephalosporins (47.5 %) and in general to all betalactamics, with the exception of piperacillin / tazobactam (36.1 %). The best sensitivity result corresponded to carbapenemics. Conclusions: the strains showed high levels of resistance. Piperazillin / tazobactam and meropenen were the best therapeutic options. The main resistance mechanism to betalactamics was the presence of extended-spectrum betalactamases, and for aminoglycosides, the presence of acetyltransferase (AAC (6´)).
