Anesthesia in the polytraumatized patient is maybe the greatest challenge within the specialty, on treating very severe children without having most of the times the personal pathological history and not knowing the exact magnitude of the injuries suffered. There are also important differences between the adults and children having accidents. The detailed evaluation and the correct preoperative preparation of these patients influence significantly on their evolution. It is necessary to evaluate immediately the permeability of the airways, the efficiency of ventilation, the hemodynamic state, and to make a neurological examination and to start with the volume administration. The details of the injuries, the treatment received before and the personal pathological history should be known, too. An updated bibliographic review of the main aspects that should be taken into account during the preoperative in these patients is made.
