The preterm labor is one of the main problems of the current obstetrics since it represents a frequent obstetric complication of pregnancy and it is a leading cause of perinatal death and also of long term sequelae in the survivor. It is propose that it is multicausal; although evidences support that the uterine infection is the more frequent one. High levels of interleukins 1, 6, 8 and the tumor necrosis factor alpha have been detected in the amniotic fluid of pregnants with uterine infection and idiopathic uterine dynamics, the authors made a review of the relation between pro-inflammatory cytokines and the cause of preterm delivery subclinical chorioamnionitis and also, to analyze if these may be used with a diagnostic character in these entities. Thus a bibliographic review was conducted in databases like Pubmed, Scielo, Lilacs and Hinari. According to the consulted bibliography, the pro-inflammatory cytokines are responsible for the triggering of the term labor and the preterm labor as well considerably more increased in this last one due to being almost always in association with a subclinic infection, the interleukin 6 in amniotic fluid is the better marker of intra-amniotic infection related to the preterm labor and the premature rupture of membranes. We concluded that the determination of pro-inflammatory cytokines in amniotic fluid and mother serum could be useful in the early diagnosis of the histological chorioamnionitis and in the prevention of preterm labor.
