Introduction: congenital diaphragmatic hernia is the most common developmental abnormality of the diaphragm. The term is most often used in Bochdaleck hernia on the left side of the diaphragm (75-90 %), it also refers to other less frequent variants. Congenital diaphragmatic hernia is seen 2-5 per 10,000 births. The inheritance pattern is multifactorial. Objective: to highlight the importance of early diagnosis of congenital malformations by ultrasonography in primary health care. Methods: we report the ultrasound prenatal diagnosis of a case that showed a cystic image, next to the heart in a longitudinal section of the chest, at 20 weeks of gestation. This image follows the heart beating in a «gastric dance», what was suspected a possible birth defect at the diaphragm, with no other demonstrable alteration in this time of pregnancy. Result: prior genetic counseling, the couple decided the termination of pregnancy. Conclusion: the case was confirmed by pathology as a left CDH.
