The results of 86 patients of 60 years of age or older and those of 239 under 60 years-old patients subjected to laparoscopic cholecystectomy were studied and compared in terms of surgical findings, mobi-mortality, conversion index, associated diseases, incidence of complicated cholelithiasis, surgical time, stay at hospital and post-operative recovery. The incidence of associated diseases was higher in aged patients, being cardiovascular (60 % vs 23.8 %) and gastrointestinal (22 % vs 17.5 %) the most frequent affections. Over 50 % cholecystectomies in both groups pesented some difficulties in surgical dissection according to findings (88.2 vs 61.2 %). All the perioperative complications were solved without resorting to conversion, there was only a conversion (0.3 %) in an aged patient and there was death in any group. The incidence of complicated choletithiasis was higher in the aged group (36.2 %) and the most frequent was the subacute cholelithiasis (22 %). Laparoscopic cholecystectomy took a longer surgical time in aged patients (63.4 vs 56,5 %) but the difference was not significant. Once they were operated on, stay at hospital lenght was similar in both groups averaging 24.7 hours. Mean recovery time was 8 days for patients under 60 years of age and 11 days for patients of 60 years and older. Both young and most aged patients can benefit from laparoscopic cholecystectomy since it shows a low incidence of complications, short lenght of stay at hospital and quick recovery.
