Abstract

Four patients with big pneumocephalus, two of them were tension pneumocephalus. In three cases, this entity was secondary to trauma: two caused by road accidents and one resulted from hygroma removal complication. It occurred in the fourth patient after diving into the sea. The diagnosis was based on cranial radiographies and computerized tomography. Two patients were conservatively managed. A third case underwent a tension subdural hygroma removal but the patient neurologically worsened because of a bifrontal pneumoencephalus and was treated with double subdural catheter system and intracranial pressure control. The patient´s course was favorable. The fourth patient showed a substantial neurological deterioration, underwent unilateral craniotomy on the suspicion of possible basal dural fistula. The patient died from respiratory complications after experiencing some neurological improvement. Other authors´ criteria about intracranial physiology of pneumoencephalus along with our own opinions are set forth.
