A retrospective and descriptive study of the patients operated on of complicated duodenal ulcer by Highly Selective Vagotomy (HSA) at "Dr. Carlos J. Finlay" Hospital was condcuted from 1984 to 2002. The main purpose was to show the results obtained in two variants of this technique. 211 patients were operated on and 192 of them were evaluated. 31.2 % (60 patients) that had stenosis needed a drainage procedure (duodenoplasty or pyloroplasty). Hill-Baker technique was applied to 116 patients, whereas Johnston-Wilkinson was used in 76. The average surgical time by operation was 134.5 minutes. Early postoperative complications were observed in 36.7 % of the patients with predominance of diarrheas. There were relapses in 6.8 % of the cases. The results according to Visaick were satisfactory in 90.6 % of the patients.
