The case of a 67-year-old patient with leiomyosarcoma of the left spermatic cord is reported. The patient presented a general syndrome and a solid tumor of approximately 2 cm in the spermatic cord. He underwent inguinal orchectomy with high ligation of the cord associated with radiotherapy and chemotherapy. A year later, the patient has not had any problems related to this disease. Due to the rareness and aggressivenes of this type of tumor, a review of this uncommon affection was made.
