The cellular immune response was studied in 14 patients with atypic cutaneous leishmaniasis (ACL), a clinical variant reported in Central America, and particularly in Honduras and Costa Rica, during this year, in 16 patients suffering from localized cutaneous leishmaniasis (LCL); and in 32 blood donors at the Immunology Laboratory of the National Diagnosis and Reference Laboratory (NDRL) of the Ministry of Health, in Managua, Nicaragua. The quantitation of total T lymphocytes (CD3) and the lymphocyte subsets (CD4 and CD8) with specific monoclonal antibodies were studied by indirect immunofluorescence. No significant differences were observed between the values of CD3 and CD4 in patients with ACL and normal controls. However, there was a marked increase of CD8 cells. On the other hand, it was found an important decrease of the levels of CD3 and CD4 lymphocytes in patients with CLL, but not of the levels of CD8, in relation to normal controls. Significant differences of the levels of CD3, CD4 and CD8 cells were observed between both clinical forms of the disease. It was discussed the importance of the inmunodepression of CD4 cells in CLL and the possible different behavior of T cells in ACL, with normal levels of CD4 cells and on increase of the cytotoxic subpopulation in a new and less aggressive clinical variant of the disease
