The case of a 64-year-old man that was admitted due to thoracic pain, dyspnea , asthenia, anemia and mild leukocytosis is presented. In the chest X-ray, it was observed a pleural effusion on the left hemithorax, with tumoral mass in its superior third, findings that were confirmed by CAT. The study of the pleural fluid evidenced the presence of plasmocytes and of a monoclonal peak in the zone of the g globulins. A monoclonal increase of these proteins was also detected in serum, whereas 41 % of plasmatic cells were found in the bone marrow. Based on these elements, a multiple IgG myeloma was diagnosed and chemotherapy with melphalan and prednisone was started. The patient died a year later as a result of the disease progression
