A review of a series of papers published by different authors in the last 2 decades showing evidence of a positivity about the presence of antiphospholipid antibodies in young patients with ictus of undefined etiology was made. It was also found a history of higher frequency of secondary episodes in those patients with these antibodies than in young patients with ictus without them. The anticardiolipin antibodies and the lupus anticoagulant have been the most studied so far, but antiphospholipid antibodies with other specificities that may be more specific immune markers for ictus of undetermined cause in young adults are also being reported. It is necessary to go deep into the study of diverse possible antigens of the antiphospholipid antibodies, their specificities and pathological mechanisms to have a useful phospholipid immune marker in the diagnosis of young patients with ictus and with the presence of these antibodies.
