A review of the dyslipidaemias, specially of hypercholesterolemia, that contribute substantially to the development and clinical expression of coronary atherosclerosis., was made. Considerable evidences suggest that the reduction of the serum levels of cholesterol is capable of stabilizing atherosclerotic plaques and of reducing the occurrence of plaque rupture, with the subsequent decrease of cardiovcascular events, including mortality. The process of destabilization of the plaque comprises various factors, such as endothelial dysfunction, morphological characteristics of the lesion, hemodynamic factors, inflammation and thrombosis. Thrombosis precipitates the reduction of the coronary blood flow and originates the clinical manifestations of ischemia. Lipoproteins have an effective participation in all the complexity of the atherothrombotic process. The lowering-lipid treatment diminishes the cardiovascular risk by mechanisms that possibly go from the stabilization of the atherosclerotic plaque and the attenuation of inflammatory activity to the restoration of the endothelial function. The modifications of the lipid profile by diet intervention, changes in the lifestyle, or by the use of lowering-lipid drugs, allow the reduction of the thrombogenic risk.
