The relation of the thrombotic cardiovascular risk associated with fibrinogen, that is today together with C-reactive protein, lipoprotein (a) and homocystein, a group of emerging atherogenic risk factors predicting risk for cardiovascular diseases, is analyzed .The fibrinogen represents an independent risk factor for cardiovascular diseases, established on the basis of numerous prospective and epidemiological studies.. However, the causal action of fibrinogen has not been proved yet. Clinical evidences show that the addition of fibrinogen as a risk marker may improve the stratification of risk, including patients with acute coronary syndrome. There is a range of mechanisms capable of establishing a direct relation between fibrinogen and the clinical events. The changes in the lifestyle and the pharmacological treatment are effective to reduce the fibrinogen levels. Likewise, a larger number of studies are necessary for a better understanding of the role of fibrinogen in the increase of cardiovascular thrombotic risk
