Introduction:

Changes in lifestyle can prevent many people from developing hypertension and the incidence of cardiovascular events. Hypertensive patients should be urged to adopt healthy habits, especially if they have other risk factors for noncommunicable chronic diseases.

Objective:

To design a care strategy for hypertensive elderly people in the community.

Methods:

Intervention study, contextualized development in the Southern Polyclinic of the Municipality of Sancti Spíritus in the period from 2016-2017, with a universe of 110 elderly people with arterial hypertension and 41 nurses, had four stages. In addition to the collection of information, structured interviews were carried out, review of documents: Kaff and Lawton scale in individual clinical records.

Results:

Significant changes were achieved in the level of knowledge of nurses, at the end of the training program obtained 80.49 % which shows an acceptable level. 80, 90 % of the health status of the elderly with hypertension reached a good state of health; the care of the elderly was able to confirm that 77.27 % received acceptable care.

Conclusions:

The care strategy for hypertensive elderly people in the community starts from the problematic situation of the high incidence of arterial hypertension. Actions and educational activities are articulated that favor the nurse-patient-socio-cultural context relationship, to take the care to a systemic conception that favors the monitoring and control of the disease.
