In order to evaluate the efficiency of a new system to plan health actions at the primary health care level, a descriptive retrospective study was conducted at a family physician office where a pilot study is being carried out. With the new system, the health team makes planning supported by a card designed to that end. It was preferred to evaluate its efficacy starting from the evaluation of the follow-up of the hypertensive patient before and after the introduction of this method. All the medical histories of these patients were studied at the family physician office. The office and field visits were analyzed and those in which the hypertensives were controlled were counted. The following elements were taken into consideration in each case: the type of control (office visit of field visit), the compensation or not of arterial tension, whether the decompensation was followed up, and wheter not at the end of each control the next one was notified. Finally, a comparison was made between the previous stage (2 1/2 years) and that following the establishment of the new system (7 months). With the new planning method it was possible to know the considerable increase of the number of hypertensives diagnosed at the family physician office. 100 % underwent at least one control in the period and the follow-up of the decompensated patient was improved. It was concluded that the new health actions planning card is an alternative more efficient than the present traditional method in the follow-up and control of the hypertensive patient.
