We proposed ourselves to identify the fundamental guidelines for the diagnosis of achalasia and to show the usefulness of the new services established in primary health care. A patient with dysphagia that was seen at the office in July is presented. It was possible to made the diagnosis in an early and fast way thanks to the resources available at the polyclinic: medical history, radiology, endoscopy, as well as to the the coordination with the National Center of Endoscopic Diagnosis to perform the manometry that confirmed the initial provisional diagnosis.
