It is presented a case of agenesia of the opponens of the thumb, a very rare affection, of bilateral symmetrical form, and isolated in a 5-year-old girl, who also suffers from agenesia and hypoplasia of the long flexors of the left and right thumb, respectively. A bibligraphic review was made and some considerations were expressed.
