A descriptive and retrospective study of 21 patients with the diagnosis of superior oblique palsy was carried out at the Service of Pediatric Ophthalmology and Strabismus of "Ramón Pando Ferrer" Ophthalmological Hospital from September 1999 to September 2002. The following variables were analyzed: etiology, predominant symptom, ocular motility, Knapp's classification, and effectiveness of the surgical technique applied. The most common etiological cause was the congenital, accounting for 90 %. The predominating symptom in the congenital palsy was hypertopia, with 83.3 %, whereas in the acquired palsy it was vertical diplopia, with 44.4 %. The typical disorders, such as torticollis, chin depression and facial asimmetry with 100 % were observed in the congenital palsy. This pattern was not found in the acquired palsy, and good results were attained in almost all the patients.
