The objective of this study was to evaluate the efficacy of combined therapy with timolol and dorzolamide compared to timolol and pilocarpine. Thirty eight patients with primary open-angle glaucoma were randomly treated with 0,5 % timolol and 2 % dorzolamide (n= 19) or 0,5 % timolol and 2 % pilocarpine (n= 19) so as to analyze intraocular pressure (IOP) declines, effectiveness of treatment for 4 weeks, and local and systemic adverse effects. The initial mean IOP in the group of patients treated with timolol and dorzolamide decreased from 22,84 ± 1,77 mm Hg to 18,24 ± 1,84 mm Hg after 4 weeks of treatment, p. 0,01 (reduction of 4,60 mm Hg 22,14 %). Initial mean IOP (without treatment) in the other group treated with timolol and pilocarpine was reduced from 23,06 ± 1,29 mm Hg to 19,07 ± 1,23 mm Hg after the fourth week of treatment, p. 0,01 (reduction of 3,95 mm Hg/17,31 %). No significant differences (p > 0,05) were found between both lines of treatment and were equally effective in lowering IOP. The quality of life of patients, who received dorzolamide as adjunctive treatment, was higher; pilocarpine dosing lowered and there were no side effects such as driving and reading restrictions or ocular pain. Five patients (26,31 %) and two patients (10,52 %) mentioned bitter taste in mouth and conjunctival irritation respectively in relation to dorzolamide. In the medium-term, intraocular pressure was reduced with the application of dorzolamide as well as Pilocarpine combined with timolol. Dorzolamide showed lower interference in the quality of life of patients than Pilocarpine
