Cataract surgery is considered a fast and safe procedure in experienced hands, thanks to the good outcome in most patients. It has become an increasingly popular refractive surgery. However, there are also complex cases, which require a surgical intervention that is always a challenge for the surgeon, among them is high myopia, in which the calculation of the intraocular lens is not exempt from an unexpected post-surgical refractive result, a refractive surprise. The formulas for the calculation of the lens to be implanted in high myopic patients deserve personalized attention and the treatment options for the refractive surprise that may occur in them depend on the particular characteristics of the patient and advantages or disadvantages, risk and benefit should always be evaluated. We present a patient with these characteristics, high myopia for cataract surgery of the second eye and refractive surprise in the operated eye.
