Since the goal Healt for all by the year 2000 was set up, it was made a callenge to all countries to reorient their health care systems on the basis of these objectives. It is necessary to know the health needs and ideals of the population to render comprehensive health services hat meet these needs and that be efficient as regards costs, transforming the predominant assistance character of these system and turning them into services with a preventive and rehabilitative character so necessary for the attention of the elderly and the disabled, in short systems of universal coverage and equity. To achieve this it is indispensable to change managerial approaches and methods, as well as to have financial funds. These are some of the dilemmas the sector has to face nowadays.
