Changes occurred at the end of the last millenium both in Science and Technology and in globalization and economic policy do not exclude medical sciences, which leads to a more adequate utilization of resources to meet the expectations of users, physicians and patients who are more and more demanding. This article suggests the mission, vision and strategic projection of the director and his administrative team in their roles as leaders of the entities included in the study as well as their strategy for the management by objectives, with the aim of carrying out the institutional improvement by using as tools the principles of management for the permanent improvement of overall quality or excellency, the Economic Resolution of the 5th Congress of the Cuban Communist Party and Law 187 of August, 1998. The most essential aspect is the efficient fulfillment of the mission of the institutions, paying special attention to the selection of managing cadres, scientists, technicians and staff in general as well as their motivation, training, permanence, material and moral incentive, the granting of awards for good results, all of which will allow to encourage technological innovation and give a higher added value to products and services at lower cost. This paper also analyzes the roles of the trade-union and other political and mass organizations in their struggle for the permanent improvement of quality on the basis of mutual respect and cooperation, and the analysis of problems with a team philosophy to achieve a participatory management with the active involvement of the workers in the fulfillment of the vision and mission of institutions. Self-financing of research projects, productions, services; the economic control of resources with the implementation of a cost system; the creation of intangible goods derived from copyright protection; the management of intelligent information and the development of the computer science and of telecommunications are of vital importance. Also, the interrelation of the whole institutional organizational structure without ignoring the determination of functions and assignment of responsibilities, which means teamwork, is fundamental.
