Introduction Smoking is the main preventable cause of death worldwide. The World Health Organization estimates that 5 million deaths occur annually and if this present consumption tendency continues, then this figure could double in a short term. Objectives To estimate the smoking-attributable mortality in the Cuban population and provide necessary information to carry out effective public health actions. Methods A descriptive design using smoking prevalence and mortality rates data in 1195 and 2007. Three causes of death groups were formed: malignant tumors, chronic cardiovascular diseases and chronic respiratory diseases. Etiological fractions and attributed mortality were calculated by cause and sex. Results Of the deaths occurred in 1995 and 2007, 15% and 18% of preventable diseases were attributed to smoking, respectively. In Cuba, smoking caused 86% of deaths from lung cancer, 78% of deaths from chronic obstructive pulmonary diseases, 28% of deceases from ischemic cardiopathy and 26th of deceases from cerebrovascular disease in 2007. Conclusions Smoking is responsible for high preventable mortality in Cuba. The country relies on the willingness of the administrative and political authorities and on more than half of smokers to quit smoking whereas it firmly advances to materialize sustainable actions to achieve results that may contribute to increase life expectancy at birth and the quality of life of the people, aware of the fact that reducing smoking is the most effective step to decrease preventable morbidity and mortality.
