The individual and social health determinants play a primary role in the development of non-communicable chronic diseases. The medical care for these determinants goes beyond the health promotion launched in the mass media. A systematic and customized procedure is required to provide the individuals with necessary knowledge to change their lifestyle by taking a keenly aware attitude to face situations that might cause non-communicable chronic diseases in a short- or long-term. The introduction of these concepts into the social practice could represent a substantial contribution to the welfare of the population and to the attainment of more effective and economically sustainable public health. The objective of this paper is to implement a methodological instrument in the social practice to persuade individuals within the community to make necessary changes in their lifestyle in order to keep and to improve their health status. Additionally, this instrument will allow systematically evaluating in a more reliable and accurate way the level of awareness as well as the concrete results of changes in the lifestyles. The viability of this methodological instrument is undergoing a trial period in practice. The results of this works show the feasibility of the project called “Individual and social health determinants, one year after the implementation in “Tomas Romay” university polyclinics in Habana Vieja municipality, La Habana province.
